Breast augmentation doesn't have to completely change your natural breasts. Many people just want a bit more volume, better balance, or to bring back fullness while keeping the softness and movement they already have. Motiva Preservé lets Dr. Alexandra Hart use a tissue-preserving method that protects your natural breast tissue and keeps the chest muscle untouched. At her Atlanta office, Dr. Hart recommends Preservé when your anatomy and goals make it the best choice for a natural look.
What It Is: A tissue-sparing breast augmentation technique designed to preserve more of the breast’s natural tissues and support structures during implant placement.
Implants: Motiva SmoothSilk® Ergonomix® silicone breast implants, designed for a soft feel and natural movement.
Implant Placement: Above the pectoral muscle so the chest muscle remains undisturbed.
Key Benefits: Smaller incisions, less tissue disruption, natural-looking breast enhancement, no general anesthesia required, and potentially a more comfortable early recovery.
Best For: Selected patients undergoing primary breast augmentation who have enough natural breast tissue to provide good implant coverage
Recovery: Many patients return to lighter daily activities within about a week, with strenuous exercise added gradually as healing progresses.

Motiva Preservé is a breast augmentation method that protects more of your natural tissue by changing how the implant pocket is made. Instead of wide surgical cuts, Dr. Hart uses special Preservé tools to gently separate and expand the tissue, making space for the implant above the chest muscle. This approach helps keep more of your breast tissue, ligaments, nerves, blood vessels, and support structures safe. It isn't right for everyone, so Dr. Hart checks your breast tissue, chest wall, fold under the breast, skin quality, breast symmetry, and the size you want before deciding if Preservé is a good fit for you.
Much of what sets Preservé apart happens inside the body. Before surgery, Dr. Hart measures your breasts and chest, picks an implant that matches your shape, and plans where the implant pocket will go. During surgery, she uses special Preservé tools to make that space with less cutting than traditional breast augmentation usually requires.
Through a small incision near the inframammary fold, the Channel Separator creates a pathway into the planned implant space. It allows Dr. Hart to reach the correct tissue plane while minimizing disruption to the surrounding breast tissue.
An inflatable balloon is then placed through that pathway and gradually expanded. Rather than cutting out a larger pocket, the balloon gently displaces the tissue to create space that matches the surgical plan.
Once the breast pocket is ready, the implant is placed through the incision using an insertion device that reduces direct handling. Preservé keeps the implant above the pectoral muscle, so there is no need to elevate or divide the chest muscle.
The tools used in this technique help make it possible, but they don't decide the final result. Dr. Hart still needs to match the implant size, pocket position, breast width, soft-tissue coverage, and your natural anatomy. Careful planning and Dr. Hart’s experience are key to getting the best outcome.

Motiva SmoothSilk Ergonomix implants are made of silicone gel and are designed to feel soft and move like natural breast tissue. The flexible gel changes shape with your body, looking rounder when you lie down and more like a teardrop when you stand up. For many people, this means their breasts look and move naturally instead of staying in one shape.
The outside of the implant has Motiva’s SmoothSilk surface. Your body always forms a layer of scar tissue around any breast implant, and the surface of the implant can affect how your body reacts. Studies have looked at how different surfaces change inflammation and scar tissue, including the risk of hardening. Motiva made the SmoothSilk surface to lower this response.
For the right patient, Preservé can offer several advantages:
Preservé isn't always the better choice for breast augmentation. It's a different method, and it works best when your anatomy is a good match for the technique.
With Preservé, Dr. Hart creates the implant pocket above the chest muscle using controlled tissue separation and expansion. Traditional breast augmentation gives her a broader range of options. Depending on your tissue thickness and breast shape, an implant can be placed above the muscle, below it, or using a dual-plane technique. That flexibility can be especially useful for thinner patients, women with limited natural tissue coverage, patients who need more substantial reshaping, or those undergoing revision surgery.
Dr. Hart offers both methods. She'll recommend the one that helps you reach your goals while protecting your tissues and giving you stable, natural-looking results.

Some patients come in wanting noticeably fuller breasts. Others simply want to replace the volume they have lost after pregnancy, breastfeeding, weight loss, or aging. Preservé can address either type of goal when the amount of enhancement and the patient’s anatomy are a good match for the technique.
Dr. Hart chooses the implant size and shape by looking at your breast width, chest wall, natural breast shape, existing tissue, and your body’s proportions. She can also use the implant choice to help balance your breasts if one side is different. The goal is not to pick the largest implant possible, but to find the right amount of volume for the shape and fullness you want, so your results look natural on your body.
Preservé is generally best suited to healthy patients having their first breast augmentation who have enough natural breast tissue to comfortably cover an implant above the muscle. It may be particularly appealing if you want a moderate increase in breast volume, natural movement, and an approach that minimizes disruption to your existing anatomy.
You may be a good candidate if you:
If you have thin breast tissue, sagging, past surgery, uneven breasts, or want a bigger change, you can still have breast augmentation. It just might mean Dr. Hart can get you a better result with a different surgical method.

An implant can restore breast volume, but it can’t remove loose skin or reposition a breast that has dropped significantly. If pregnancy, aging, or weight loss has changed both the fullness and position of your breasts, Dr. Hart may recommend a breast lift along with augmentation.

Many people like Preservé because it causes less tissue disruption and keeps the chest muscle untouched. This can make the first part of recovery easier for some patients. You should still expect some swelling, tenderness, pressure, and tightness in your breasts after surgery. Dr. Hart will give you instructions about your support bra, how to care for your incision, your medication, how to sleep, and what activities are safe.
Most people can go back to light daily activities in about a week, depending on their job and how they feel. This does not mean your breasts are fully healed yet. You’ll need to wait for Dr. Hart’s approval before doing heavy lifting, upper-body workouts, or hard exercise. Swelling and tenderness will keep getting better over the next few weeks, and your breasts will slowly soften and settle.

You’ll notice the extra breast volume right away, but at first your breasts will look higher, firmer, and more swollen than they will later. As the swelling goes down and your tissues relax around the implants, your breasts will look softer and more natural. Most people can expect this settling process to last for several months.
Silicone breast implants can last a long time, but they’re not meant to last forever. Pregnancy, weight changes, aging, and changes in your natural breast tissue can all affect your breast shape over time. Dr. Hart will explain the recommended check-ups and long-term care for your Motiva implants so you know what to expect.

No. Motiva Preservé is the tissue-sparing surgical technique used to create the implant pocket. Motiva breast implants are the silicone gel implants placed inside that pocket. The technique and implant work together, but they are not the same thing.
Yes. The FDA approved Motiva SmoothSilk® Round and SmoothSilk® Round Ergonomix® silicone gel-filled breast implants in 2024 for breast augmentation in women aged 22 and older. Dr. Hart will discuss which implant options are appropriate for your anatomy and breast enhancement goals.
Motiva SmoothSilk Ergonomix implants contain a soft, flexible silicone gel designed to respond naturally to movement and body position. They tend to look rounder when you are lying down and take on a more teardrop-like shape when you are standing, which can help create a natural look and feel.
The technique is designed to minimize disruption to breast tissue, nerves, and other structures involved in sensation. That may help preserve nipple sensation, but changes in breast or nipple sensation remain a possible risk with any breast augmentation surgery.
Preservé can restore volume and may improve mild deflation, but it cannot remove a significant amount of loose skin. If your breasts have dropped on the chest, Dr. Hart may recommend adding a breast lift or using another breast augmentation technique.
The cost depends on your implant selection, the complexity of your surgical plan, anesthesia and facility needs, and whether your breast augmentation is combined with a breast lift or another procedure. After your consultation, Dr. Hart’s team will provide a personalized quote and review available financing options.
